  1. ethernet uses only 2 pairs, if you use POE the other 2 pairs are uses for poe.

it's not recommended to send 24vac and data on same line. it will probably work but def not the right way to do it.

you can get not-too-pricey POE injector and put it near the camera. that model you have there supports POE so that would be my personall preference if I were you.

  2. if you can access the DVR locally in the LAN and the DVR loads up your problem is with your port forwarding.

check this how to config your router http://www.manualowl.com/m/Netgear/WNDR3400v1/Manual/220076?page=87

once completed both needed ports go to canyouseeme.org and check if the port forward worked.

also make sure your other network setting on the DVR are correct, Subnet and gateway. (255.255.255.0/192.168.1.x) if these are missing the router will have problems forwarding the requests.

  3. what does your windows log says?

That will be where you will find the exact reason.

my guess is one of your RAM sticks is bad if it only crashes on high activity.

also try putting page file on another partition than your OS install.

but again before you start swapping hardware learn how to use the event log to find your problem.
